List of Necessary Documents for Application for Certificate of Eligibility

2. Color photo of the applicant (4cm×3cm)

3. Return envelop with stamp (\430 stamp, if the “Address in Japan” on the first page of the application form is Kanagawa Prefecture. \380 stamp, if the Address is outside of the said prefecture (i.e. Tokyo, Chiba, Ibaraki, etc)

4. Supporting documents
If a college student whose student status is undergraduate, master, or doctor
（１）Copy of a letter of acceptance: nyuugaku kyoka shou (status, department, etc. must be specified)
＜A letter of acceptance is issued only after the completion of enrolment procedure; therefore, the original notice of acceptance: gougaku tsuuchi shou (status, department, etc. must be specified) needs to be submitted first, then submit the letter of acceptance as a additional document. ＞
（2）Documents certifying financial ability (either a, b or c)
a. In case the applicant himself/herself supports his/her own tuition and living expenses, 　　a certificate of scholarship or bank balance certificate under the applicant’s name
needs to be submitted

b. In case someone outside Japan (not including the applicant) is supporting tuition and
living expenses by bank transfer (both documents below are necessary)
・　 Declaration of financial support prepared by the financial supporter

・　Bank balance certificate under the name of the financial supporter

c. In case someone living in Japan is supporting tuition and living expenses
・　Declaration of financial support prepared by the financial supporter
(Download “Declaration of Financial Support”)
・　Certificate of tax withholding at source (gensen-chosyu-hyou) from the financial 　　　supporter or bank balance certificate under his/her name
If you are a research student (Category No.2 or 7 Research Student)
（1）Letter of acceptance: nyuugaku kyoka shou
(Detail/subject research must be specified)
（2）Documents certifying financial ability (either a, b or c)
a. 　In case the applicant himself/herself supports his/her own tuition and living expenses, certificate of scholarship or bank balance certificate under the applicant’s name needs to be submitted

b.　In case someone outside Japan (not including the applicant) is supporting tuition and living expenses by bank transfer (both documents specified below are necessary)
・　Declaration of financial support prepared by the financial supporter
